after 6 days of posting, firstly i have become disoriented to day.. i keep forgetting wat day is it! and secondly, to my great horror, e hospital (u know where) i am in is COMPLETELY different from ttsh!!!!!

1. ttsh: iv drips are primed and prepared in e treatment room, so all u do is swab and connect at e pt's bedside. (i dunno bout IV antibiotics, but i guess those are prepared in e treatment room too)
also, they don use stoppers.

here: i have to stuff e IV antibiotics, syringe and needle and N/S for flushing, swabs, e white and yellow stoppers and dunno wat into e kidney dish, which is really messy and crowded, and take them to e pt's bedside! at least we can take things one by one in e treatment room.. like tt very hard to remember to take everything needed..

somemore, they don use burettes for all antibiotics lor. it is a bit complicated. first they connect e bottle of powder into a 100ml pack of N/S, then they squeeze e NS into e bottle, dissolve, and squeeze back into e pack, ready to drip. next they use another 50ml pack of NS to prime e line, unspike and spike into e prepared pack of antibiotics. and after finishing e antibiotics, have to unnspike and spike back into a new pack of 50ml NS to flush. i am a bit dizzy from all e spiking and unspiking.. i hate IV antibiotics!=/

and somehow i have this impression tt e plugs come out v easily! honestly, i was taking out e baju and i was quite careful.. e sleeve slipped out of e arm easily, and i didnt feel any resistance at all, but to my horror e plug came out! but e other nurse said tt she saw e plug looked readily dislodged in e first place la..

2. writing report style is different. personally, i find APIE style a bit extra. basically, u identify a nursing diagnosis and write in e APIE format. so things like CXY done or blood despatched are not written tog in e nursing report=/ ma fan.. and then u have to scan thru e whole paragraph of APIE just to pass e main pt..

3. ttsh's staff lounge is definitely bigger than here.. ok i am being fussy! hee.. spoilt.. but it is really very very small here..

4. i am so used to ttsh's way of NGT feeding tt i dunno how other hospitals do it.. i think ttsh swab so many times cuz they got VRE rite? anyway, i think ph indicator is not v useful cuz they hardly got aspiration..

5. i feel tt alot of things tt ttsh implement has got their rationale behind it and they communicate it to their staff.. here i don really understand wat is going on la, to be honest.. even e routine.. like for eg, give med first, then check drips, then wound dressing? they prob have la, but it is not as obvious i guess.. or i haven found out yet.. i dunno..

i am a bit scared tt i will breach some confidentiality issues by writing bout their systems=/ hope not.. anyway, in conclusion, i feel v disorganised and messy in my ward (for many reasons, some not stated).. u might say tt cuz i am not used to it yet, ya maybe.. but i think perhaps this time it is not so much an adapting problem, it is a preference prob.. it is not hard to adapt, as in i can do it their way, but in my heart i'll still have my own preference..

and i didnt expect my T to P to be like tt.. i really don feel closely guided leh.. and i don understand y they put me and yy in diff shifts when we share e same CI? and nurses in e ward don seem to know we are doing T to P lor.. they asked us if we are in yr 1 or yr 2..
